Hi S.
Ouch- I had something similar- but only one wrist thankfully with twins. I had to eventually have surgery but held off for some time.
It sounds like you have some incentive to lose beside taking back your health! Yay. Sometimes that's what it takes because while the journey is worth it- it's not easy.

You can also look it up at thinkslim.org too
I'm on call this week- if you email it might take me a bit to send you info if our schedule is busy in the OR.

first of all forget the narcotics-then get rid of all junk food-switch from pop to water-propel is awesome-eat more fruits an veggies,i really wouldnt suggest sit up or push ups at your current weight-start with long walks-put baby in stroller an just go out walking- much more productive at your current weight-also cortizone shots in your wrists will do wonders for carpel tunnel...had it done..relief was immediate.also take a multi vitamin-i take ALIVE xtra energy-when you eat it boosts your metabolism-gives you awesome energy.you really need to change your way of thinking fast-your severe obesity will only worsen your health problems.you want to live long enuff to see your baby grow up dont you?..walk walk walk...water water water-flush out your system...good luck

Are you on the depo shot by any chance? MY DIL just went off the shot because it made her gain over 100 lbs, she went on Mirena instead. If you are using gthe depo find another birth control, the weight loss after going off the shot can be quick.
To get started ---- you need to get going. Walk, even if it's only a few blocks at first. Park your car away from a store to walk into the store.
Wrist rotations help a lot with carpel tunnel. I had double carple tunnel several years ago and went to a chirpractor who told me when it's in both wrists--it is often caused by a pinched nerve in your neck rather than the wrist.
I just saw an add on TV last night that Weight Watcher's has a special 'join for a $1' campaign going on now.
Stay off any drugs --- it's like any other issue drugs mask the emotions. Sometimes you have to face the emotions, take them out, shake them up and tell the bad ones to leave. ------- Not easy I know, been there.
The biggest thing you need to know is you can do this. Once you decide you can --- you will.

See a chiropractor about the carpal tunnel. Check out Nutritional Weight and Wellness http://www.weightandwellness.com/ for information about changing what you eat to help with the FM. They recommend cutting out all processed sugars, and eating more proteins and good fats. They have helped lots of people lose weight as well as control numerous diseases, especially ones involving inflammation.

I know you are in IA but you could still probably arrange a consultation or they could recommend a good nutritionist in your area. Start slowly by getting dietary changes in place. Then start with swimming because it is easy on joints and connective tissue. Just walking in water can burn a lot of calories. Drugs are not the answer, they will just complicate your health problems.

Please get your thyroid tested, tsh, T3 and T4 are the needed tests. Carpel tunnel is a symptom of low thyroid (hypothyroidism) and will slow your metabolism. All your problems could be fixed fairly easily if it is your thyroid. (no narcotics needed then) If it is your thyroid, do not take the generic version of the thyroid meds. Only name brand like Synthroid, ,my whole family has hypothyroidism and generics do not work for us or many other we know.It is a simple blood test and demand it is tested. Good Luck.
PS google TSH level range, because some labs are using and old range from 0.5 to 5 is normal anything over 5 is hypo (low thyroid function) in case you are told you are normal, you might not be if the lab is using the old range. Really a TSH of 3 or over is low thyroid function.
