Maybe a silly question, but I am drawing a blank on what to write on a birthday cake, other than "happy birthday!" The person is mom, grandma, great-grandma, and aunt to the people at the celebration...so the typical "happy birthday -insert name- doesn't work since everyone calls her something different! Any creative ideas?? or should I just go with "happy birthday"

Use her given name, a nickname, or just say "Happy 90th birthday," "90 years young" or something like "You're the best."
You can also skip the writing since everyone knows who it's for, and put on one of those photos of her in a happy or funny pose.
Use her first name...happy birthday Marie! Or happy birthday beautiful!
Does she have a pet/nic name she goes by? That's what I would use (what we have done in our family).
I would use her name...Happy Birthday Betsey! We love you!
We had this dilemma for my grandmother's 100th birthday. We had her picture on it and just happy birthday. She was tickled to see herself on the cake!

Just Happy Birthday and her real name. I've made cakes for my grandmother and grandmother-in-law for significant birthdays where to everyone in attendance the guests of honor had different titles but at the end of the day, they were still "Mary" and "Anne" so that's what went on the cakes.
It depends on the "style" you're going for. If you wanted to really have fun with it and it's a big enough cake, you could write "Happy Birthday" in big letters and then list her various "names" underneath in smaller letters, similar to what you might do if you gave her one large birthday card from the whole group. "HAPPY BIRTHDAY mom ~ granny ~ great gran ~ aunt sue"
Otherwise, I think just "Happy Birthday" or a photo as some suggested below, will be great.
"Happy Birthday Marie" or just plain "Happy Birthday!". I also have family pet names but the cake always says "Happy Birthday N." and when my family sings happy birthday, then at that point they all yell out whichever nickname they have for me (in the "happy birthday, dear ____" part of the song), so that makes things easier for everyone, including the cake decorator, but allows them to rely on personalization when singing and of course, when handing out a birthday card or gift with said nickname on it.
I think just happy birthday is fine. Or you could put happy birthday to an awesome lady.
