Gold or silver (a ring or jewelry) or illustrated books or small feeding set. Maybe cash instead of the traditional gold or silver?
This is the traditional and symbolic first feeding of rice. (A book will symbolize the feeding of the mind.)

My husband's family traditionally gives various pieces of gold jewelry for babies' significant milestones. That is nice, but it is not very useful for us, and all of it has been packed away until they are older.

If the parents are very traditional, they MIGHT go ahead and have their child wear the gold jewelry (I have seen many Indian children in my neighborhood wearing gold bracelets, necklaces, etc.)

wow! Thanks for teaching me something tonight! I had to google this event....what a wonderful milestone to celebrate.

As for a gift, I would recommend hitting a search engine for items which are meaningful during this ceremony. Maybe that would help you choose a gift.
