Messy Underpants Before Going Poop

My daughter turns 5 in a few weeks. She potty trained at 3.5 and while she's done great with #1, #2 has been a bit of a struggle. We've had to deal with her not wanting to use the potty to do #2, being afraid of the potty, getting constipated from holding onto it, all of that. Feeding her a high-fiber diet and sometimes giving Miralax on a semi-regular basis (i.e. every other day) has helped and she's doing much better now. She may still only go #2 about 3 times a week but she doesn't have any difficulty and she goes on the potty willingly on her own. If we try giving Miralax more often, she ends up with diarrhea several times a day - it's like there's no happy medium. The poop is typically huge and often the toilet will get clogged up when she tries to flush it. But just before she's about to have a poop, she will dirty her underpants - sometimes just once, sometimes 2 or 3 times. It's just a very small amount of poop but I wonder if she's is just waiting too long to go, or if there is something else going on. Has anyone else experienced this with their kids at this age or even later? Anything we can do to help her realize she probably needs to get to the potty sooner before her panties get messed up? TIA!

If she is going that much so that it clogs the toilet then she needs help. You should talk to your pediatrician. The messy panties is because the loose stuff is coming out around the big blockage. I think this might be what is lubing the big one so that it can come out. Its always been my understanding that a person should go once a day about the same time everyday, and if they don't then there is something wrong. Going 3x a week is too little!

My pedi said that once it hurts to go they will hold it in. He said to give my 2.5 year old 2 doses of miralax every day (full adult doses) until she has 2 or 3 bowel movements every day for at least 2 weeks. This will get her used to going and it not hurting. There will probably be a few accidents, but it will be better for her in the long run.

I would call your doc and talk to him, or take her in, but this is NOT normal and its not OK. You don't want her to end up with IBS or some other issues later in life if you can stop it now. Good luck mama!

My daughter now 4 used have that leak issue, and it was exactly what Marla S. said. And your daughter may just think it is gas... my daughter would tell me she tooted. I finally got her to "feel" the differnece between an air toot and a liquid one... kinda gross but...

Anyway, what helped with her was a higher fiber diet and probiotics. If you have it by you Lifeway makes Kefir smoothies in large bottles and also kids squeeze packets with nifty names like Goo Berry Pie and Lime Slime.

She has that every other day or so and now she goes every day at around 1:30 and depending on how much she is eating again 7:30 ish.

You need to talk to the pediatrician about this and get referred to a pediatric gastroenterologist who will work with you and a nutritionist. The issues aren't being solved with this cycle you have going on and it's time to bring in some professional advice. I went through something similar with my autistic daughter.

Take her to a Pediatric Gastroenterologist.
They see this kind of thing, everyday.

Withholding their poop, causes pain. Then it becomes an emotion based, issue, as well as a physical issue. 2 issues. This is per the Ped. Gastroenterologist, we saw.
My daughter went through that, even if we did not "force" pooping, it was just her and just the thought of pooping, caused stress and anxiety for her.
It took 3 months, for the vicious cycle to ease.

Leaking of poop, can be "Encopresis." And you do not want that to happen or get worse.
Withholding poop, can also cause internal problems. And constipation. And bulging bowels etc.

You need the guidance of a Specialist.

Perhaps it's time for disposables (pull ups again) and I would stop the over the counter meds and replace with natural (such as prunes, juice, oatmeal, etc.).

I would also have her sit on the potty two or three times a day (when she gets up in the morning....after meals...and before bedtime)...I would have her sit at least 5 to 10 minutes....give her a little juice/water and then a book. Sometimes when we just relax that's what it takes to have a regular movement.

Ditto Marla S., she said exactly what I was going to say.

If your daughter's poop is so large it's clogging the toilet she isn't going often enough, you need to get her system flushed out so she can go normally. The poop holding caused this, unfortunately it doesn't just disappear but leaves residual problems, her body needs help. The leakage messing her undies is trying to lubricate the poop to come out more easily. My little guy did it for awhile (he was a major poop holder when he first trained) and his ped helped me get him cleaned out so he can go normally, he has been going every day like clockwork, recently with the heat it has been every other, but he's not clogging the toilet anymore. Also, I do have IBS and know when I get like this I'm backed up, too, and have to take measures to get on track.

Please contact her pediatrician Monday morning, you will more than likely need to increase the frequency of her Miralax for a couple of weeks, diarrhea or not she needs to get flushed out so she can get to normal, poor little dear.
