Have you spoken to the manager at the care facility? They might have a suggestion as they have other residents who may be in the same situation.

Dr. Les Hata in Oakland (College Ave. near Broadway) is excellent - has a very calm, gentle, and patient manner, even when my twin 7YO's are underfoot during my checkups he keeps his cool, and both my kids have no problems with him checking or cleaning their teeth, even the one who is on the mild end of the autism spectrum and can easily get panicky/upset over other things.
