Hello,

My daughter was born at Baylor Grapevine and I no longer have the paperwork the hospital gave me to request a copy of her birth certificate. Does anyone know where I need to go to get a copy?

Thanks and God Bless!
AH

Bureau of Vital Statistics. In Dallas it's located at the Records Building. In Tarrant county not sure, but the county clerk can tell you or you can find it on line.

You can contact the county where she was born. The county should have a vital records office and you can generally get the order form online or even place the order online.

You can get them from the Bureau of Vital Statistics. And now you can order them online http://www.dshs.state.tx.us/VS/reqproc/certified_copy.shtm I just ordered my daughter's online. They said it may take a few weeks, but we got hers three days after we placed the order. Online ordering wasn't available when my son was born, so it sure beat waiting in line!!

Any place that issues birth certificates in the state of Texas can issue the certificate. I just got mine in McKinney and was born in Hurst.
