Sun screen (spray on and face rub on)
Lip Balm
Bug spray
Swim diapers
Rash Guard/Swim suit
Towel
A change of clothes to something lighter
A frozen teething ring (in the diaper bag) - I use this for my 3 year old when he gets too hot. He loves it.

I do the same thing, I have a tub in my trunk that I keep swim gear in and a change of clothes. I actually got this together this past weekend.

I also put in
neosporin (neo to go)
neosporin boo boo cleanser
bandaides
Benedrly to go (for bug bites)
Hand sanitizer - for when the hand wipes just don't cut it.
One time use or throw away ice packs.
These all depend on the age of your child. It they are at the walking, running stage these things are essential. Cause boo boo's are gonna happen.
I also use Avon's Skin so Soft with Bug Guard. This way you have two in one and you can also use it. I have these in my van, in the bag, hanging on the stroller. This way if you run out or it gets taken out of the bag you have back up. I also keep small bottles of water in the bag. Perfect size for little hands and it doesn't take up alot of room. I also put wet washclothes in zip locks for the bigger messes and there is no where to clean up. Extra clothes for accidents. Now that my daughter is alittle older I keep a toothbrush and toothpaste in her bag. I also put in alittle pouch with Advil for us. My purse always seems to be empty. Plus all the other things that other posters put. Yeah my bag is packed but better safe then sorry. Oh and if we go somewhere where we need a cooler I put a boo boo bunny in the ice. My daughter always asks for boo boo bunny when she gets hurt.

Sunscreen
Hat
Sun glasses
Extra fluids
Layering items (in case it's chilly at night)
** When my son was young I actually kept most of these items in a little canvas tote in my trunk b/c that darn diaper bag gets to heavy!**

This is what I keep in my 1yo diaper bag. Diapers, wipes, one outfit, pacifer wipes(for toys he doesn't take a paci) a snack cup,a sippy cup,a couple cars and a small ball, sunscreen(when it gets warmer we aren't out that much yet) emergency pad and tampon for myself. I use a small diaper bag but it is packed full. I keep a first aid kit in my car so unless we walk to a park I don't generally keep any first aid things in the bag. I keep hand sanitizer in my purse. If we are going somewhere buggy I grab the off and stick it in the bag but I don't take it everywhere.

I also carry neosporin bandaids and Off. They are helpful for outdoor fun.

I always have for my three month old diapers, wipes, those little wrist rattles, a book, extra bottles and a bib, hand sanitizer, a hat for baby, a pad for the changing tables, diaper rash cream just in case, whatever I would normally have in my purse (I pretty much don't carry a purse when I have him with me and just put everything in the diaper bag. Now that the summer is coming I have sunblock and bug spray. I actually used it yesterday when we decide to take my son on a trail walk through a Lincoln exhibit. Also, I always pack extra clothes and now sunglasses in case it gets to bright outside.

All of these are great, I make sure to have extra plastic bags to put dirty diapers or wet clothes in. Also stain wipes, to pretreat stains before washing them, you don't always get to that right away!

The staples for my 16 month old sun and 5 and 6 year old girls:

sunscreen
bug spray
sun hats
sunglasses
extra towels
bottles of water
snacks that won't melt (like pretzels or goldfish)
diapers
wipees
hand sanitizer
benadryl (okay, that's for me bc I'm allergic to bee stings)
toilet paper and/or paper napkins
little toys or books
straws (for MY drinks, LOL)

I think those are the most basic, bare minimum things I keep on my at all times. The sunscreen, stroller, pack and play, bug spray... most of this stuff I keep in a tote in the car at all times, so I never have to worry about forgetting anything!

I got a diaper bag when I was prego... and have only used it three times. LOL! I have a little diaper kit that always has 3 diapers and wipes in it... In the car I keep a change of clothes (for DD AND me!), jacket, baggie of nibbles, couple of toys, and sunscreen. That's it. I have never needed anything more.

I love Pampers clean and go, they don't smell and are in a small package to put in the diaper bag or car.
A bottle of water and a juice box
a snack
band-aids
hats
sunglasses
watershoes/sandals(just in case you go somewhere you end up splashing)
sunscreen
extra clothes
I bough a little fan at wal-mart that has foam blades, that way if a baby/child touches it it won't hurt. I always carrry two so if it is super hot we can cool off fast.
I usually put a small box in my car to toss things in, like sand toys, old cars to push around at the park, a beach towel or our pinic blanket. My son is three so I don't normally carry a diaper bag. I do have a backpack all packed and ready so where ever we go I am prepared. In the backpack I have a small coin purse that has about five dollars so if we are out and the ice cream truck comes buy or we need a super cold drink I have it.

Oh my! I stopped carrying a diaper bag when my youngest of two turned one! I usually try to remember to shove a diaper and some wipes in my purse but I have spares in the car.

It really depends on the age of the child. Sunblock is a must have. Freeze the childs juice and water so it will stay cold and refreshing, and keep them in an insulated carrier so they stay cold and won't melt real fast. Always no matter what age you need a change of clothes.
