4Yo Traveling & Visiting with Grandma Out of State - Documents Needed???

My four year old son will be traveling (airplane) and staying with his Grandma and Poppa out of state for a short vacation this summer.
My son already has a Passport for when he is traveling on airplane with his grandma (since they have different last names).

I believe we need a power of attorney form to state that my mother has care over my son when he is with her as well as his medical insurance card (in case of an emergency).

Does anyone know the easiest way to obtain this form and/or is this the appropriate form we need to obtain for my son while he is in her care.

Also look into this:
It is called "Child Travel Consent Forms" or
"Affidavit of Parental Consent" .
It has to be notarized.

If you Google Search it, you can find free templates of it.
Such as:

We used this when my Husband traveled with my Daughter.
Our Travel Agent said it is required. Some airlines etc., will ask for it.

I ditto SH. advice below...but we have never had our documents notarized.

We include a copy of the parents passport and all contact info with the written consent for the child.

This has worked for international travel as well.

When my kids went with family on vacation last year, I just typed up a form stating who had permission to obtain medical care for them. I put a copy of their insurance card with the form. I also typed up a list of their doctors & dentists with their information, any medications the kids took and dosages / times, my information, and any medical history that might be needed. That way it was all together and my inlaws would have it in an emergency. I just figured that this would get them emergency care and I could be there in a couple of hours if needed.

you should be able to just write a letter stating that you parents have permission to take care of any medical needs get it noterized at your bank as well as a copy of your drivers license also noterized. if they have any questions beyond that they will contact you.

You can get a general Power of Attorney online or through an attorney.

But, as long as they are not traveling internationally, a young child doesn't need ID or POA. You can call your airline to verify what (if anything) is necessary for your child's flight.

I never made a power of attorney. I just wrote a permission letter stating that my parents could seek medical treatment for my kiddo. I also sent our medical insurance card as well. That is really all you need. Hope your son has a wonderful visit!!!

You should probably contact an attorney to ask this question. I don't think you need a "power of attorney", but a "Medical Proxy" or something similar. In case of emergency, she should call an ambulance and have him taken to the ER where they will treat first and "call" after.

Make sure that she has a copy (front and back) of his insurance card and yours! Also make sure that she has the name and number of your son's pediatrician and any specialists who work with him.
