My son had one when he was a little guy as well. I used a natural product that my aunt makes - baby butt balm http://www.rangelandherbs.com/lotions.htm. Also I have heard that the Arbonne products are good. I have a friend that has a 3 year old that has suffered from bad diaper rash since he was an infant and she swears by zinc oxide. Baby's skin is so very sensitive... have you tried not using baby wipes and using a warm damp wash cloth?

Rule of thumb, my favorite NICU nurse told me...if the diaper rash is going on longer than three days call the doctor. I couldn't let it go that long, so I called the doctor after my son's first one at three months old appeared and just based on my description the nurse called in a prescription.
Even if you get a prescription, try the following:
Let him be naked and air out. I agree with the sun time! My son's bootie cleared up after just one day of fresh air and no diaper. At night, I would put down one of those pee pads to protect the bed and wrap him in cloth diapers...it worked pretty well.
Good Luck!

Take him to his doctor to rule out a yeast infection.
