Could It Be Diaper Rash??? Help!

Hello knowledgable ladies! My stomach is suddenly in a knot this evening and I may even call the pediatrician. My 3 year old girl (still not potty trained, it's a power struggle) has complained about her 'butt' hurting for QUITE some time. I finally realized about 2 weeks ago that she had a really bright red vagina and put some desitin diaper rash cream in it. It seemed to go away. Just a day or 2 ago it came back. Then this evening she had a very messy dirty diaper and I decided to really 'get in there' and make sure she was clean - on the wipe I used there was a pinkish spot/color like maybe blood. I gasped, she cried, it was over; clean diaper. Then just before bed she sat on the potty to tinkle and bawled about her butt hurting. I asked her to show me. I laid her on the floor, pulled things back a bit to look and almost cried. She is kind of bloody in a pink sort of way in that spot between her vagina and actual anal hole. (sorry to be graphic!) I am HORRIFIED. What in the world could have caused this? There is no possible way she could have been abused as she is under my constant supervision, literally 24/7. I don't believe it's yeast because the desitin wouldn't have worked 2 weeks ago - right? Could it be diaper rash? Should I wait for her 3 year physical in 2 weeks or take her to the doctor right away? She doesn't have a fever and the complaints of her butt hurting seem to happen after she pees. Could bladder infections happen w/o fever? There's no bizaare odor to speak of - should I worry or relax?!?!?!? Help, I'm panicking over my baby!!!!!!

So What Happened?

Hi Gals, thank you SO much for your input. After posting my request last night I called the nurse line, then this morning had my daughter soaking in a warm bath with baking soda in it while making an appt w/her pediatrician. We were in first thing this morning. They cathed her and couldn't find a UTI, any infection, bacteria, or anything. They're looking at the culture overnight. It's not even yeast! My poor DD, with her overly sensitive skin, simply reacted to something, be it diapers, food she ate, or just the bubbles and baby wash I was using. We're trying a non-steriod perscription ointment that is safe for her p****** p****. I am frustrated that it was simply her sensitive skin flaring up but relieved that it isn't major; she is happy and told her daddy this evening that she 'had owies on my booty but the doctor made it better!" I think the ointment has already begun to work. Thanks so much for all the encouragement!

I do feel you should take her in very soon because it does sound like a yeast infection- my oldest, which was a long time ago had it where I also felt- "oh, it's better" and then it came back with a vengeance. Hers was a result of being on an antibiotic a few weeks prior. A lot of people told me that the way to tell the difference between that and a diaper rash is that the yeast infection is such a bright red.

Side note: If you have problems with bladder infections(my kids are quite sensitive to bubble bath)- one glass of cranberry juice and no need for the doctor!

Take her to the doctor. She has to be very uncomfortable.
I have 4 children 2 girls 2 boys. I have dealt with diaper rash but nothing like what you are descriping. If you don't have it looked at it could get worse.

I didn't read through your answers but this is so familar that I thought I would mention it. My granddaughter seems to be allergic to something in some brands of disposible diapers and pullups. It makes a rash on her as you are describing. While the diaper rash cream seems to help it some, it still gets pretty sore if we don't use it each time. We changed to a different brand and all seemed well though. She is now in panties and loves the potty training. She is just over 2 years and hasn't had an accident for a week or so now. I would just put panties with rubber pants over the top of them and tell your daughter she has to so her bottom will heal. Use corn starch and baking soda baths to help. Use wet wipes to clean her so she doesn't have the scratchy tp. I bet when she realizes how much nicer it is to always be dry she will do better with the potty training. I used andie mints as rewards when she would potty in the potty chair and she does better with a seat on the toilet then the potty chair. I also put the potty chair in the livingroom while she was here, at least the first week or so to keep her from having to leave her play for as long and it helps with that hurry we have to go part until she is able to tell before hand that she needs to go. That only took a few accidents which she hated the feeling of going down her leg or filling the plastic pants before she was going in the bathroom and going on her own. I wouldn't fight your daughter on it, just put her on there when you go to let her know it is ok. Give her a special book or something to read because at times it takes a few minutes of relaxing. We used the sunday funny pages. It took my granddaughter only a week to go from us asking and taking her in to try to potty to her telling us she has to potty because she was ready and we never got after her over accidents. Last time she had an accident she was upset for a minute then said "it was just an accident, it is ok".

I really do believe that your daughter could have a allergic reaction to the diapers or pullups though. It is worth a try.

Have you asked your daughter if she has been scratching at the area? Maybe she has irritated it on her own trying to make it feel better. I would definitely call your doctor and try to get her in for an appointment asap. I would not wait for her next scheduled appointment. You don't want her to be in any unneccesary pain for that long. Try not to worry!

This happened to my daughter. She would get the worst rashes, and then it moved to her vaginal area. We were given perscription creams which we used daily for close to a YEAR before finally realizing she was ALLERGIC to disposable diapers. Have googled the chemicals they put in those things? It's absolutely disgusting. She was only one when we figured this out and switched to cloth diapers (the kind that work just like disposables - one piece, velcro), and we never had a problem after that. Take her to the doctor for sure, but rethink the disposables.

To me it sounds like a diaper rash- one of the acid burns that eats the skin. This tends to happen when urine and poop mix. I would suggest you let her "air out". Allow her to go around with nothing on her bottom for a while (works well under a longish skirt)- as long as she agrees to go potty in the potty :) And, of course a good bath.

Explain to her that you think the problem is because she is still wearing diapers and she has to use to potty if she wants it to go away. Make potty training her choice for her happiness.

If it doesn't start looking better after airing out a day or two- then I would schedule to have it looked at.

It could be a urinary infection. My son had one once and he didn't run a fever. The only reason I knew he had one was because he was still in diapers and I noticed his urine was dark and smelled bad when I changed him. If I were you, I would take her in now. Better safe than sorry.

I wouldn't wait two weeks, I would take her in now. Speaking as a woman, anything out of the ordinary can be uncomfortable down there and I would never want my daughter to go longer than she has to without treatment.

Hi R.,
Do you know if your daughter has been constipated or having hard, larger stools? If so, she might have an anal fissure. It is a tear on the lining of the anus. It can start to heal, but then get torn again. What is her diet like?
Otherwise, I'd call the doctor's office and ask to talk to a nurse/nurse practitioner for advice. They should be able to tell you whether you can try something to help the condition at home or whether to bring her in.

Sounds like diaper rash to me, but you might want to have her checked for a bladder infection. When my daughter was diaper rashy the doctor told me not to use any bubblebath...even the ones they say are safe for kids, that they're really harsh. Good luck!

don't worry!!!
i have seen this so many times as a day care provider....
usually in the 18-24 mo old range when they are trying all kinds of new foods
fruits usually were the culprit (high acidity)
a few days of desitin usually helps however, i have been introduced to 'bag balm' which is available at farm and fleet and used for cow udders/teets to prevent cracking and bleeding. the few times i encountered this bleeding diaper rash on my own daughters (4 and 18 months), it cleared it right up i use it for everything (including my dry hands).....
for the struggle it is to potty train your three yr old; she can she doesn't want to, right?
i'd try to convince her the pain she is feeling is because she sits in a poopy diaper and some foods she will have to stop eating until she is potty trained and poops in the toilet. then pick some of her favorites and see if that gives her some incentive....seriously though if you just eliminate oranges, pineapple, and tomato based things, it should stop bleeding like that.... hope that helps ya.... becky

My 2 year old just had something like this too and i freaked out as well...got the last appointment at 4:55 that day and it was yeast infection..not sure if that is what your daughter has but it sounds very similar. We treated it with a antifungal cream mixed with Maalox and Vaseline. it really helped, we did that 4 times a day and then for a week after it had cleared up...another thing she said is that no body wash/hair wash in the bath with her that wait to wash her body & hair at the very last part of the bath as the water is draining. I guess with little girls it isn't good for them to sit in anything but just the bath water..another thing you could do is put 4 tablespoons of baking soda in her bath if it seems to be really painful. Hope this helps!!

Hi,
You might want to try to apply some plain Organic yogurt. When my kids had diaper rash, I just sat them down in some plain yogurt (that had warmed up a bit to room temperature otherwise it's a shock if it's cold).

It's cool, whatever is in the yogurt calms the burning and if they try to eat it, so what, it's good for you.

Just let her sit in it or put some on and let her run around for a few minutes before gently wiping some of it off. (you can leave a little and then diaper).

ALSO, be ware of the soap you are using, Don't use any type of deoderant soap, try some gentle soap from the health food store.

I would say clean her up really good use a Blow dryer on LOW to dry her butt and let her run ard for as long as you see fit free also use vasilene(spelling)(when she has diaper on) it creates a moistire barrier good luck

It sounds like a diaper rash to me. My neighbors son had a bloody diaper rash between one diaper being fine and the next being a little bloody and raw. I would call the doctor because you might just need a RX for a stronger diaper ointment. She got poop goop from the dr and it cleared it up in hours.

As long as she doesnt have a fever that is good. Sometimes kids get an infection from their poop getting in their vagina. Its very common when potty training. My daughter had gotten a fever for just one day but had the same thing. I did take her to the doctor and they put her on some meds. They said it is very common. A vineger bath or a bath with no soap helps also. I would take her in because she is so uncomfortable and you dont want to wait till she gets a blatter infection which is very painful.

If she is that uncomfortable, take her to the doctor. It sounds like it is a diaper rash of some sort and the skin has become very red and raw and has opened up. That is where the blood has come from. Having open skin in that area is a breeding ground for infection if there is not one already. I would take her to the doctor right away and not wait until her well check. Call and see if the office can get her in ASAP and see if they can do her well check also so you don't have to make two appts. Hope this helps.

I think you should take her into the doctor. She probably has a Urinary Tract infection or something similar. That can cause the vagina to become really red and irritated, and if it is irritated and she is scratching or itching at all, she can cause herself to bleed. I wouldn't worry too much but I would bring her in right away. You can get infections without fever and anything can bring them on. It could have been the bath or not wiping enough. Nobody's fault at all.

I have three small children and also work for a dermatologist. Our youngest son had this problem - blood too - simply on his scrotum. It was determined that it was indeed contact dermatitis from his diapers. We have since switched to cloth and no issues whatsoever. If the rash becomes inflamed it can cause infection. We used a multitude of Rx creams, one of which was antifungal and that cleared it up. If your PCP cannot determine the cause, I'd explore the possibility of visiting a pediatric dermatology practice. A great deal of pediatricians simply are not able to diagnosis diaper-area rashes quickly enough, telling patients that the over-the-counter creams are best. Most times, particularly in the case of infection, they are not. That particular area of the body is very sensitive and with consistent moisture, and exposure to everything that is found in paper diapers, is prone to rash and infection. Best of luck!

Diaper rashes can easily turn into yeast infections. A simple thing to use is the Lotriman antifungal (for athlete's foot) over the counter medicine. You need to use the original cream, not any of the others. I had heard this before and the doctor confirmed it for me. Also, if there are open sores, use some Neosporin to keep it from getting infected. Girls' privates are very sensitive and get irritated VERY easily.

I don't mean to come off as rude, but if it were you that was bleeding in that area and you cried each time you went to the bathroom cuz it hurt, would you wait two weeks to go see your doctor??
Take her in. If you are "horrified", this shouldn't even be a question that you would post on Mamasource. Desitin is not going to cure her. She needs a doctors care.
If you did wait another two weeks, the doctor is going to ask you "how long has she been like this"? Your answer is going to be "about 3-4 weeks". Doesn't look too good, does it?? Take her in now!

I don't have girls, but if your doc doesn't come up with a concrete answer, I would seriously consider changing your diapering plans. A good friend of mine who is in no way an "earth mama" type had a daughter who got constant yeast infections and sores from disposables. I guess it's a ton more likely that girls will have problems from disposables than boys, so it's probably something to consider if your doc doesn't come up with anything. good luck!

I would take her in to the doctor tomorrow. Yes, you can have a bladder infection without a fever.

Other things to think about are: has she had any new foods or drinks lately or been exposed to any new soaps or laundry detergent?

To help with the skin irritation, she might find it soothing to sit in a bath some Epsom Salts. Also, the absolute best diaper cream we used is Weleda's Calendula Diaper Cream. You can get it at co-ops, Whole Foods, and Peapod's (St. Paul--Snelling & Randolph). Calendula is very soothing and mild.

I just saw your "what happened" and glad to hear it's nothing serious. I would stop using bubble bath altogether. My boys had sensitive skin and couldn't tolerate any bubble baths, even the ones for sensitive skin. We couldn't use the "bath paints" either. We use either colloidal oatmeal products or Cetaphil in their baths. Many bubble baths also contain warnings that they can cause vaginal problems. Good luck!

Could be the yeast infection just like we get at times. Check with your Dr of course, but I have had a preschool for teachers children for a number of years and there has been several times moms have brought a cream for yeast and it cleared it right up.

Sounds like yeast, but you should take her in to be diagnosed, and to make sure she doesnt have a UTI, as well. One of my dd's gets yeast infections when she eats too much bread and/or sugar, it is common. Keep her bubbles out of her bath, too, that is h*** o* her skin. Take a deep breath, call the doc, and take her in. Chances are it is yeast and they will help you with that and let you know what to do if it happens again.

I would definitely take her in as it sounds like a yeast infection. In addition to the anti-fungal cream you should get, our doctor suggested that we stop using diaper wipes until it cleared up as they are so drying. We used Mustela PhysiObebe no-rinse cleansing fluid on a washcloth. Your laundry load will increase a bit but it cleared up quickly with the cream

If you are this worried you need to call your doctor right away! It could just be diaper rash, or it could be something else. It shouldn't be bloody. That is scary! I would try to get her in as soon as possible. Good luck!

There isn't a need to panic, but I would call your nurse practitioner and see if they want to see her or consult you over the phone. It is possible that she has a urinary tract infection, bacterial infection or a yeast infection. She wouldn't necessarily have a fever or foul odor and Decitin won't cure it. This happened to my daughter and they recommended an anti fungal cream for the yeast infection. My daughter is very prone to infections, so when she was 3, I started bathing her standing in the tub and at age 4, she just started showering. If she sits in the tub for any length of time with even the mildest baby bath/ shampoo mixed in the water, it's over. She has an infection. Not everyone is this sensitive, but my daughter is.

From what I know (and have experienced) Destin will NOT clear up a Yeast Infection. My daughter had one and we were prescribed an anti-funal cream - same as what is used on athelets foot.

I would recommend bringing her to the doctor. Sounds like she is in alot of pain...

It actually could be yeast. The desitin will soothe it teporarilly but will not get rid of it. If you take her in they will probably give you a triple paste. Thats what they gave my daughter when she was having the same problem. Good luck and I hope she feels better.
