Afraid Something Is Wrong in First Trimester

I went to the ER for severe morning sickness. I am breastfeeding an 8 month old, and my period never returned. At the ER i was given 2 bags of fluid and zofran. He also done a quick ultrasound and said I was 8 weeks.My OB nurse saw me when my prescription ran out, exactly 2 weeks later. She done an internal exam (no ultrasound) and said i measure 8 weeks. Shouldnt the baby have grown? She also couldn't find a heartbeat with a doppler. Is this normal? I am very afraid something happened to the baby.

So What Happened?

I have no idea on a date. I am still breastfeeding and never got a period. The ER did not check blood hgc because I had no idea when I got pregnant, he just checked to see how badly I was dehydrated. That is where I was told I was 8 weeks. Then the OB nurse said 8 weeks based on an internal exam with no ultrasound two weeks later. That is what worries me the most. I'm guessing that this early on the ultrasound was pretty close, so wouldn't the baby have grown in those two weeks? She did do blood work, and I go back on 12/12. Feels years away. No spotting or bleeding at all, just severe nausea without the zofran

*All she said was try not to worry, and that I will the the senior Doctor at my next visit. She said he would do an ultrasound to check the baby. I know thy are busy right now, but I really don't like the wait and see approach. Tomorrow is 2 weeks since my last visit. I have been having a hard time not trying to get fit in earlier, but 12/12 is a long ways off at this point. Should I call and try to be seen earlier?

My doctor didn't want to try to find my baby's heartbeat with a Doppler at 12 weeks because he didn't want to panic me if he couldn't find it. You are still not that far along. Best of luck!

Measuring the yolk sac with ultrasound and measuring your fundus (height of your uterus)with a manual exam is apples to oranges. It does not mean the baby didn't grow. As someone who has lost three first trimester pregnancies, when something did not measure correctly after week 8, I was sent to the hospital Radiology department for a STAT ultrasound. What did they suggest as the next step? Don't let the manual exam get to you, they are not accurate. I myself measured two weeks bigger than normal with my first baby and they thought I had twins so sent me off for an ultrasound. Baby measured on schedule, I measured big (I showed early, carried huge and had a lot of amniotic fluid). Just because your uterus measure small does not mean the baby measures small. You may have less amniotic fluid, and some woman just carry small.

Are you very thin? If not, I would not expect to hear heartbeat with doppler.
The nurse isn't an ultrasound technician, her measurements could very well have been off.
I wouldn't worry!

I'm sorry you're sick.....a Doppler cannot pick up a heartbeat til ten weeks at the very earliest, with most babies heartbeats being heard at twelve weeks. Do not worry! When a fetus dies, the uterus expels it, so you would have bleeding. Your baby seems a ok! An internal exam at 8 or 10 weeks can be off significantly. Go by the ultrasound. In early pregnancy it's very accurate. I hope this info eases your mind some. Good luck and congrats on 2 bundles of joy!

With all 4 of my pregnancies heart tones were detected very early in pregnancy via heart doppler,internal/external ultrasound.Are you off on date of conception?When will you see your OB next?Did they do lab work see what your HCG levels are & if so were they in normal limits.Are you cramping spotting/bleeding

I was always told that if you're that sick you're producing pregnancy hormones and the baby is fine. I never heard a heart beat for either of my sons until I was closer to 14/15 weeks along--and the doctor was never that concerned about it. I was so sick I figured everything was fine. I don't think they can tell very well how far along you are just by an internal exam so I wouldn't be concerned about the inconsistency there.

Measurements are not exact and I wouldn't worry about the heart beat not being heard either. I know people who heard it very soon and some not as early and they had healthy babies. Don't worry because that makes you sicker with stress and everything else to deal with. Just wait and try to get your doctor to do an exam and ultrasound at the next visit. Ask any and all questions when you see him.

I wouldn't panic over the different in size. You can't compare what one person says from an ultrasound to what another says from measurements.

However, the lack of fetal heartbeat by doppler at 8 weeks entitles you to an immediate ultrasound to make sure the baby is ok. You are justified in calling the office and insisting on the ultrasound now.

The ultrasound should be quite accurate for the date at that stage. A physical exam is much less reliable , so I wouldn't worry at all about that. Also, the first trimester the baby is too small for them to get a blind Doppler of the heart rate, so I wouldn't worry about that either. I bet everything is just fine.

With my experience with my miscarriages and pregnancies, when I thought something was wrong, there was. Just trust your instincts. But the fact that you are having severe morning sickness should actually be a good sign! With my last miscarriage, I just knew something was wrong and I wasn't having any nausea or anything which worried me. And 8 weeks is very early to hear a heartbeat with the doppler. I am currently expecting my 4th child and at my 10 week appt, the Dr said she would check with the doppler, but if we didn't hear it not to panic and she would check with ultrasound. She did find it right away!

I would call, ask to speak with a nurse. She can give you your hcg levels and address your concerns. They deal with this all the time.

I would call your OB office and let them know how worried you are. Ask if you can please just come in for an ultrasound for peace of mind.
